Required Documents for US Citizens
* A copy of official and sealed transcript. This transcript is not required for visa processing in Korea. You may need a transcript copy for Korean Consulate/Embassy which you would contact for the actual visa stamp on your passport. You need to check with a particular Consulate/Embassy before you send out your passport/visa application. Different consulates may have different policy for the transcript. Please also make sure to check visa fee.

Diploma
Take your Diploma and make a photo copy. Take the original and the photo copy to a notary public and obtain the notary stamp for the photo copy of your diploma. Please make sure you get the photo copy notarized and not the original because the Immigration will not return your documents back to you. Once you obtain the Notary Stamp, you will then need to send the notarized copy to the State Secretary of State where your university is located for the Apostille.
Health Statement
Please download the health statement. Please complete the form, sign the bottom and email us at your earliest convenience. The actual physical will take place here in Korea. The physical will consist of a routine physical, blood and urine test where they will check for controlled substance, STD, and HIV.

Send us 2 and keep 2 for your visa application that you need to submit to the Korean Consulate.

Please note the FBI must authenticate the Criminal Record Check. Without the authentication, you will not be able to obtain the required Apostille. Mail the required items —signed applicant information form, fingerprint card, and payment of $18 U.S. dollars - to the following address:

⁃ Once you receive the Authenticated FBI CRC you will need to send the FBI check to the U.S. Department of States in Washington D.C. Before sending out, please email us the scanned copy of the Authenticated FBI CRC at parkenglish@parkenglish.com.
⁃ You may send your FBI Check to http://www.usauthentication.com/rates.html. This is a service Park English requests candidates to utilize to get FBI check apostilled quickly. Please visit the company’s website for a detailed explanation about the Federal apostille process.
⁃ A State apostille is also acceptable. However some State’s Secretaries do apostille FBI checks, some do not. You may check this before you send out.
